ALL patients coming to the clinic for in-person appointments MUST be up to date on their COVID-19 vaccinations. Up to date is defined as having "received all doses in the primary series and all boosters recommended for you, when eligible." (CDC)
Please bring bring proof of vaccination with you to the clinic.
If you are NOT vaccinated or up to date with your vaccines, your appointment will be via telemedicine.
