ALL patients coming to the clinic for in-person appointments MUST be vaccinated against COVID-19.

Please bring bring proof of vaccination with you to the clinic.
If you are NOT vaccinated, your appointment will be via telemedicine.

We are not allowing visitors to accompany patients to appointments at this time unless medically necessary. If you require someone to come with you to your appointment, they MUST be vaccinated against COVID-19 and bring proof of vaccination with them to the appointment.

  • Medical care for children under 18

  • Emergency care - Please go to Emergency Room or call 911 if experiencing:

    • Sudden and severe symptoms​

    • Chest pain

    • Shortness of breath or choking

    • Vomiting

    • Trauma or injury

  • Specialty services (such as pediatric, dental, psychological) - for a list of services we recommend to our patients, please see our Community Resources.

  • Pre-employment physicals

  • Care for non-urgent medical needs

    • Skin rashes, urinary infections, cold, flu, minor injury, sore throat, cough, general malaise

  • Management of chronic diseases such as asthma, diabetes, heart disease, hypertension

  • Women’s health: STD screenings, pregnancy tests, obstetrical visits

  • Counseling on nutrition, lifestyle modification, medication adherence, and navigation of healthcare resources

  • Vaccinations: MMR, TDAP, Hepatitis A & B, Pneumonia, Pneumococcal, Flu

  • Follow-up medical visits
