ALL patients coming to the clinic for in-person appointments MUST be vaccinated against COVID-19.
Please bring bring proof of vaccination with you to the clinic.
If you are NOT vaccinated, your appointment will be via telemedicine.
We are not allowing visitors to accompany patients to appointments at this time unless medically necessary. If you require someone to come with you to your appointment, they MUST be vaccinated against COVID-19 and bring proof of vaccination with them to the appointment.

Care for non-urgent medical needs
Skin rashes, urinary infections, cold, flu, minor injury, sore throat, cough, general malaise
Management of chronic diseases such as asthma, diabetes, heart disease, hypertension
Women’s health: STD screenings, pregnancy tests, obstetrical visits
Counseling on nutrition, lifestyle modification, medication adherence, and navigation of healthcare resources
Vaccinations: MMR, TDAP, Hepatitis A & B, Pneumonia, Pneumococcal, Flu
